Welcome to IARC Surrogacy from Surrogacy Mentor!
We are thrilled you have chosen us for your Surrogacy Journey! Surrogacy Mentor will be sending us your pregnancy & delivery records and other information they have. We will be reviewing that information and will follow up with any questions we may have regarding those items. In the meantime, you may begin submitting the forms and documents needed for our screening process.
Forms needed for the IARC pre-screening process:
→ Surrogate Application (This application is different than the one on the main application page as this one is specifically for Surrogacy Mentor Surrogates)
→ Release of Information form (You will be getting an email from your Surrogate Coordinator to complete this form and they will be emailing you a password to access)
→ Gestational Surrogate Medical Clearance (You can send through email to your Coordinator or fax 763-367-6847)
→ Background Check – One for you and one for your spouse/significant other (if applicable)
As part of the screening process, we also need copies of the following for your file. Feel free to take pictures of the items and send the pictures if that’s easiest for you.
-copy of insurance card (front and back)
-copy of insurance booklet with benefits & exclusions
-copy of marriage certificate (if applicable)
-copy of birth certificate
-copy of birth certificate for your spouse (if applicable)
-copy of social security card
-copy of passport (if you have one)
-copy of last 2 paystub showing normal hours
-copy of last 2 paystubs showing normal hours for husband/spouse (if applicable)
– 4-6 pictures of you and your family for your surrogate profile
– Dear Parents Letter. See sample letters here.
Your Coordinator will give you regular updates as you go and guide you through the entire screening process. Let us know if you have any questions as you complete forms and gather the documents. We are always here to help!
